Lapeer County School Employees Credit Union Deduction

I authorize the Lapeer Community School to make a deduction from my net pay to be deposited in an account I establish at the Lapeer County School Employees Credit Union. I understand that no deduction or deposit will be made until Lapeer Community Schools receives confirmation of my arrangement from the Lapeer County School Employees Credit Union (you must establish an account with the Credit Union and inform them of the amount that you will deposit.) I also understand that the deduction will continue, each pay date, until I provide a written notice (to be received at least two weeks before the first paycheck without the deduction) to Lapeer Community Schools to discontinue the deduction.
